In the student's name and Number box, type the date in which you took the class . If you don't know the exact date, at least put the month or year. When checking out be sure to list the address you would like your certificate mailed to. For questions please call 503-489-2599.

If we find out that you did not take your training through our agency after our research, you will NOT get a refund. If for any reason you think you did no complete the training through us you should call first. All Sales Are Final.
